Kokoromil Inc. YAMANASHI! Pilot Program Support Project (YAMANASHI!
LIFEUP Project)" led by Yamanashi Prefecture.In this pilot program, a convenient long-term screening test was conducted during daily life, successfully detecting 31% of arrhythmia risks and 36% of signs of sleep apnea syndrome (SAS).Furthermore, the usefulness of a next-generation, digitally-enabled regional preventive healthcare collaboration model was demonstrated, which smoothly connects individuals identified as needing attention (Category C) from online consultations to Yamanashi Prefectural Central Hospital (a regional core hospital).■ Background and Objectives of the Pilot Program"Heart disease" ranks first in the prevalence of illness among certified care recipients in Yamanashi Prefecture. Long-term electrocardiogram (ECG) examinations are effective for early detection of heart disease risks. However, conventional ECG tests, which last about 15 seconds and are performed during general health checkups or medical examinations, have the challenge of overlooking signs of temporary arrhythmias.Additionally, for business professionals, psychological and time-related hurdles exist, such as "too busy to take time off for a checkup" and "thinking it's not a big deal."This pilot program aimed to verify a model that provides a system allowing for 24-hour testing while living normally at home or work, supporting individuals from early detection through treatment at regional medical institutions in a seamless manner.■ Pilot Program Results and AchievementsProof of High Screening EffectivenessThe program involved 94 employees from two cooperating local companies (Assist Engineering Co., Ltd. and Kofu Shinkin Bank). While the arrhythmia detection rate in general health checkups is around 10%, this pilot detected 31% of arrhythmia signs (5% or 5 individuals classified as Category C, requiring attention). Signs of sleep apnea syndrome were also detected in 36% of participants (16% or 15 individuals classified as Category C, requiring attention), significantly contributing to the early visualization of "hidden risks" of which individuals may be unaware.Reliable Medical Collaboration from Online Consultation to Regional Core HospitalOf the 5 individuals identified with arrhythmia risks (Category C), 4 utilized online clinic services. Subsequently, in accordance with the pilot model, 3 individuals were smoothly connected to specialized outpatient services at "Yamanashi Prefectural Central Hospital," achieving a 100% connection rate from recommendation to regional medical care.■ Comment from Dr. Umegatani, Chief of Cardiology at Yamanashi Prefectural Central Hospital"Early detection of arrhythmias is crucial. Particularly, many patients with atrial fibrillation remain asymptomatic and can present with sudden stroke or heart failure. By recording the ECG for 24 hours, it is possible to detect atrial fibrillation early and connect patients to early treatment. I believe this is an excellent initiative that connects simple workplace and home testing to regional preventive healthcare.
